Transillumination: the key to successful infiltration

It all depends on the surface. How deep is the lesion located in the enamel and how thick is the intact enamel layer above the lesion? Assessing this correctly is crucial for establishing the right treatment procedure. Transillumination provides a clear indication of the position of the lesion in the dental enamel. It allows you to estimate the thickness of the intact surface above the opacity and the position of the lesion and to select the right treatment procedure.

Transillumination for more in-depth diagnosis

The lesion topography is assessed with the help of transillumination before treatment begins. A suitable light source is positioned palatally and the light-optical properties of the transilluminated teeth are evaluated from the vestibular angle. Transillumination allows a meaningful view of the lesion, thereby enabling an assessment of depth, size and complexity. If the lesion is close to the surface, the lesion edges appear more sharply defined, but if the lesion is deeper, the lesion edges are more blurred.

Whether you are dealing with incipient caries, fluorosis, MIH or trauma – minimally invasive infiltration treatment with Icon Vestibular is always the most straightforward and gentle treatment option for white spots and enamel opacities. That’s because infiltration doesn’t involve any unnecessary removal of the tooth structure. This makes infiltration treatment a versatile choice for treating enamel opacities efficiently and painlessly in just one session.

The Icon Vestibular infiltration concept is very effective in infiltrating hypomineralised enamel of carious origin, commonly referred to as white spots. The one-off infiltration treatment gives the enamel back its natural refractive index and stabilises it. That’s because the innovative infiltration treatment gently cures the porous tooth and permanently seals the porous tissue. With MIH, the affected enamel structure differs significantly from the healthy enamel, and massive enamel collapses may also occur. Young MIH patients may also suffer from severe sensitivity in the affected teeth. In their 2021 study, Diago et al. were able to show that infiltration treatment with Icon Vestibular offers a minimally invasive solution, especially for children. It reduces hypersensitivity, facilitates better oral hygiene and helps prevent further dental complications. In other words, infiltration treatment with Icon Vestibular is a reliable aid in the effective and gentle treatment of MIH patients.

Example applications show that a bleaching treatment before infiltration can significantly improve the overall result. Bleaching reduces the colour difference between healthy and affected tooth enamel and removes discolouration. Especially for patients with developmental enamel defects such as fluorosis or MIH, Flairesse Bleaching Gel, which is approved as a medical product, is a gentle and painless alternative. When bleaching is combined with infiltration treatment, a waiting period of two weeks is recommended between treatments. The use of a bleaching gel containing carbamide peroxide reduces the brownish parts of organic protein structures. As a result, the teeth are visually whitened overall.

Transillumination is the perfect diagnostic aid before infiltration with Icon Vestibular, and can also be used for quality assurance during infiltration treatments. (Note: Do not use a curing lamp during infiltration.)

Transillumination before infiltration treatments literally sheds light on the darkness, because it makes the location and depth of the lesion visible in the enamel, thus minimising misdiagnoses and unnecessarily invasive treatments.

During the infiltration treatment, transillumination is an ideal tool for monitoring the success of the infiltration process and to better assess the drying stage – but be careful: do not use a polymerisation lamp for this!
